Use Open Backgrounds Folder in settings, add images or videos, then restart.
Can I use GIF backgrounds?
No. Animated backgrounds use WebM videos instead.
Can I add it mid-playthrough?

Compatibility notes
The author documents RimThemes integration. BetterLoading overrides loading-screen backgrounds, while menu backgrounds continue working. The Apply on game load setting has no effect with BetterLoading enabled. Artwork has reuse restrictions under its CC BY-NC-ND licence.
