mlieThe Plasteel Surgery Mod introduces advanced medical procedures that allow you to alter a pawn’s body type, biological sex, voice, immunity and beauty through plasteel-based surgical techniques.
NOTE: The original version is outdated as of 1.1. An updated version was later provided separately.
Plasteel Surgery Features
The Plasteel Surgery Mod adds new research and surgeries requiring plasteel, industrial medicine or better, and a skilled surgeon.
Body Modification
- Change body type to Fat, Thin, Hulking, Masculine or Feminine.
Sex Reassignment
- Change a pawn’s biological sex (does not alter body type).
Laryngoplasty
- Fix Annoying Voice, Creepy Breathing or a botched laryngoplasty.
Immunity Boost
- Remove the Sickly trait or add Super Immune.
Facial Reconstruction
- Increase or decrease beauty by one level per surgery.
- Multiple surgeries required to move across several tiers.
- Failed surgery decreases beauty by one level.
- Three research tiers: Basic, Intermediate and Advanced.

If you want to change hair or face visuals, use the Barber Pod mod.
Compatibility notes
- Can be safely added to or removed from existing saves.
- Generally compatible with other mods; report conflicts if found.
- Known issue: Changing the sex of a pawn in a relationship may cause unexpected behavior (they may remain together but stop engaging romantically if orientations no longer match).
- Includes support for modifying BB bodies (thanks HR-SP!).
- Includes Chinese translation (thanks 法师Mage!).
Support and troubleshooting
- Test whether the issue persists with only the Plasteel Surgery Mod (and required mods) enabled.
- If it disappears, re-enable other mods gradually until the issue returns.
- Always share your log using the Log Uploader.
- For best support, use the Discord channel for error reporting (discussion threads may be missed).
- If you have the solution for a problem, post it to the GitHub repository.
